What to Bring

Insurance Card

Whether you are a new patient or a returning patient with different insurance, it is very helpful for us to be able to scan your insurance card so that we can contact your insurance company to verify your benefits.


We need a prescription from your physician before we can provide any device. This prescription can come from any doctor, physician's assistant, or nurse practitioner. The prescription should include your name and date of birth, the device you need, your diagnosis, the doctor's name, and signature.


Below is the list of insurance companies we participate with. If you do not see your insurance company, please contact us. Many insurance plans offer out-of-network benefits. And we are always willing to see patients on a self-pay basis.
